Non financial Barriers to Accessing Healthcare

One of the primary reasons for underutilization of healthcare services is cost. However, Kullgren, McLaughlin, Mitra, and Armstrong (2012) comment that over 20 percent of Americans do not seek out healthcare service in a timely fashion due to nonfinancial reasons. These barriers have to do less with affordability and more with accessibility and acceptability.

Nonfinancial barriers to healthcare include lack of transportation, long wait times, and work or family responsibilities. Consider, also, issues such as health needs, language, and cultural norms. Review the article in depth and discuss some healthcare policies that could be adopted to overcome these nonfinancial barriers to accessing healthcare.

Florida State University Case Vignette Paper

Case vignette. Please read the case vignette and answer the following questions:

How might you in a subtle way to promote dependency? What personal needs might be met by encouraging members to need you?

What do you think is meant by the phrase “promote termination of members from the group in the most efficient period”?

  • Case Vignette
  • In this case, the group leader conducts an ongoing group with no termination in sight. Whenever a member wants to quit, he or she is reminded of each person’s responsibility to the group, and there is a discussion of how the person has not worked through all his or her struggles fully and is therefore not ready to terminate. The leader maintains that dependence on the leader is an expected part of the therapy process. Personality change is not necessarily accomplished in 12 months of therapy, the leader contends. This leader sees nothing wrong in having people come to the group for a long time. She gives the analogy of the person with diabetes that needs to take insulin for the rest of his or her life. She assumes that there are always issues to work through and eventually the members will be able to work through their dependency if they stay with the group for a long time and if they are willing to work through their feelings of transference toward the group leader and perhaps other members.

NURS6650 Walden WK2 Family Assessment and Phases of Family Therapy

Week 2: Family Assessment and Phases of Family Therapy

A family’s patterns of behavior influences [sic] the individual and therefore may need to be a part of the treatment plan. In marriage and family therapy, the unit of treatment isn’t just the person – even if only a single person is interviewed – it is the set of relationships in which the person is imbedded.

—American Association of Marriage and Family Therapy, “About Marriage and Family Therapists”

When issues arise within a family unit, the family often presents with one member identified as the “problem.” However, you will frequently find that the issue is not necessarily the “problem client,” but rather dysfunctional family patterns and relationships. To better understand such patterns and relationships and develop a family treatment plan, it is essential that the practitioner appropriately assess all family members. This requires you to have a strong foundation in family assessment and therapy.

This week, as you explore family assessment and therapy, you assess client families presenting for psychotherapy. You also develop diagnoses for clients receiving psychotherapy and consider the legal and ethical implications of counseling these clients.

Assignment 1: Hernandez Family Assessment

Assessment is as essential to family therapy as it is to individual therapy. Although families often present with one person identified as the “problem,” the assessment process will help you better understand family roles and determine whether the identified problem client is in fact the root of the family’s issues. As you examine the Hernandez Family: Sessions 1-6 videos in this week’s Learning Resources, consider how you might assess and treat the client family.

Learning Objectives

Students will:
  • Assess client families presenting for psychotherapy

To prepare:

  • Review this week’s Learning Resources and reflect on the insights they provide on family assessment.
  • View the Hernandez Family: Sessions 1-6 videos, and consider how you might assess the family in the case study.

Note: For guidance on writing a comprehensive client assessment, refer to pages 137–142 of Wheeler (2014) in this week’s Learning Resources.

The Assignment

Address in a comprehensive client assessment of the Hernandez family the following:

  • Demographic information
  • Presenting problem
  • History or present illness
  • Past psychiatric history
  • Medical history
  • Substance use history
  • Developmental history
  • Family psychiatric history
  • Psychosocial history
  • History of abuse and/or trauma
  • Review of systems
  • Physical assessment
  • Mental status exam
  • Differential diagnosis
  • Case formulation
  • Treatment plan

Note: Any item you are unable to address from the video should be marked “needs to be added to” as you would in an actual comprehensive client assessment

Diet Analysis Project; Summary A record of seven days of food intake will be recorded, and analysis of three of the days will be performed, using the

Diet analysis project.

Instructions; Collect seven days of food intake, and record the approximate time, a description of the food or beverage you’ve consumed [chicken-fried, milk-1% low fat], and a measured amount. The amount is the tricky part but attempt to be as accurate as you can. The measurement must be in a unit that is a common weight or volume, such as a cup, or ounce, or ½ pound. Not in units that have not specific meaning such as a bowl or glass or spoon full, or a large amount.

You will need to select three days, one of which should be a weekend day, and prepare to analysis these days. You will need to provide information on the amount of food you consumed. At some point when you are providing a summary of your eating behaviors you will need to comment on the amount you eat as a serving as compared to what the serving size is for that food.

You will then provide the calories you consumed from each food and break down the food for carbohydrate, fat and protein intake. You will also need to provide the intake of certain vitamins and minerals. If you have purchased a book that came with an access code for a web based dietary analysis you may certainly use this program. If you did not get a code with your book, a dietary analysis program will be available to you, such as the one at Mypryamid.gov. You may also use a nutrient composition table accessed on the internet of your choosing. (http://www.choosemy0plate.gov/
http://www.choosemyplate.gov/supertracker-tools/supertracker.html)

Compare your three days. Is there a wide variation for daily calories, amount of food or for certain nutrients? Include a comment about this in your summary. You are also to write in your summary about how you think your eating behaviors and your nutrient intake are meeting your nutritional needs and what you might do to improve them. For this section you may use the U.S. Dietary Guidelines provided in the beginning of your text or anything else covered in your book or in class. This is not an assignment that you are to obsess over. There is no way to be 100% accurate so do the best you can at assessing how your diet/nutrition is, and provide some critical and creative thinking on how you might improve it. Have FUN with this!
