Becoming an expert in nursing requires one to “return to” an incident or event (positive or negative) and reflect on the reasoning behind the decision-making. Through critical reflection, nurses become aware of their personal beliefs, feelings and knowledge that influence their actions.

The student is to choose an incident that occurred during the clinical day. The issue can be focused on the patient’s pathophysiology, education needs, psychosocial needs, socioeconomic conditions, or cultural/religious concerns. It can be an incident like a code blue, a procedure such as a dressing change, an interaction (positive or negative) with a family or staff member.

Need Introduction

My instructor informed me that I was able to pass medications on my patient for the first time that morning to my patient I was assigned too. Patient was kind enough to let me give her mediations as well as medication. I had to give a sub q injection of Heprain (blood thinner) in her lower abdomen. As my instructor helped me dispense medication that we had to crush because patient was unable to swallow. So we mixed them in with chocolate pudding. I as well after administering my medication did my first Head to Toe assessment as my instructor watched.

Patient was admitted on the 09/11/18 for Congestive Heart failure. She is 60 years old with a medical history of Asthma, COPD, Pulmonary hypertension, seizures and obesity. She has an allergy to aspirin. Patient uses a nasal cannula with 4-5 liters of oxygen. Her mental status was AOx4. She smokes 2-3 packs of cigarettes a day since she was 15 years old.

I followed the charge nurse Monica today and was assigned to her patient in bed 32A. Monica had a lot of responsibility as being the charge nurse such as monitoring patients throughout the shift and report any emergencies to physicians promptly; address patient complaints in a timely fashion to ensure patient satisfaction; manage daily staff assignment and patient scheduling to ensure clinic coverage and flow for instance today she was short a medical assistant and she had to make more than five calls to see if anyone wanted over time or was willing to come in help for a few hours and she was met with a No response several time still she finally had a medical assistant Carla from another floor that volunteered. Monica has to perform daily rounds to assess patient condition and distribute medications to patients; assess and address physical and emotional needs of patients; educate patients on their health condition, maintenance and treatments all of these things and she still managed time to teach me and educate me throughout the process such as showing me how to use the inhaler Spiriva. She is one amazing nurse and needs recognition. Because I am a very inquisitive student and she never seemed bother by any of multiple questions throughout the day as I shadowed her on the Pulmonary Unit.
