Ms. A. is an apparently healthy 26-year-old white woman. Since the beginning of the current golf season, Ms. A has noted increased shortness of breath and low levels of energy and enthusiasm. These symptoms seem worse during her menses. Today, while playing in a golf tournament at a high, mountainous course, she became light-headed and was taken by her golfing partner to the emergency clinic. The attending physician’s notes indicated a temperature of 98 degrees F, an elevated heart rate and respiratory rate, and low blood pressure. Ms. A states, “Menorrhagia and dysmenorrheal have been a problem for 10-12 years, and I take 1,000 mg of aspirin every 3 to 4 hours for 6 days during menstruation.” During the summer months, while playing golf, she also takes aspirin to avoid “stiffness in my joints.”

Laboratory values are as follows:

Hemoglobin = 8 g/dl

Hematocrit = 32%

Erythrocyte count = 3.1 x 10/mm

RBC smear showed microcytic and hypochromic cells

Reticulocyte count = 1.5%

Although Ennis has developed a comprehensive plan that can be applied to other calamities other than a surge in incoming contaminated victims, I find some aspects that need to be modified that can uplift the model to an optimal state.

Improvements: The model suggests that EMS providers with First Responder Awareness Level training should not be included in the transporting and treatment of the contaminated patients. I find this to be limiting for the hospital abilities to respond and treat contaminated patients. As a part of EMS providers education, the use of PPE and Decontamination process are taught. Hence, it is rather extra precautionary to limit the transporting and treatment abilities to the EMS providers with advanced training.

The Ennis model suggests choosing the most isolated areas in the hospital and designates them as the decontamination areas such as the Morgue. Then, it is suggested that an outdoor decontamination area is better. In my opinion, setting up a decontamination area in outdoor settings must be mandatory for several reasons. First, it will significantly limit infections from airborne diseases. Second, it will reduce the financial costs when the hospital decides to shut down its ventilating system. Monitoring the air quality and using instruments and experts will cost the hospital a great deal of money, while they can invest it in more training renewing the decontamination site. Third, setting up the decontamination area outside will limit the interruptions to the hospital operations, the only interruptions will be getting the staff to leave their place of work and respond to the decontamination area.

Surely the model approaches the emergency planning and response from different angles and using all the resources within the hospital to achieve its objectives. For example, the model suggests extensive training on PPE application and procedures, which suggest the collaboration of ER staff, OR staff, and health and safety department.

It is evident that protecting employees is a crucial role to play for every healthcare organization because posing them in danger will lead to interruption for the whole operation. I believe it is necessary for every hospital to have a protective measure for every potential scenario. Moreover, hospitals that are responsible for providing emergency response services must be prepared to do their work without causing any harm to staff or communities. Thus, it can be reached by establishing a solid and achievable emergency response plan to ensure stability and effectivity for emergency services delivered to people in need. To establish an effective emergency response plan, there are some aspects that need to be considered such as, preplanning which includes coordination with other healthcare organizations as well as other authorities that might influence the operation. Therefore, the implementation for an effective emergency response plan all hospitals should consider applying frequent pre-emergency drills to ensure a high level of readiness among all staff and managers. Besides, they should examine their ability in establishing the incident command system with other local emergency response organizations. As another important element in emergency response plan hospitals should have well-trained decontamination team which will be responsible for protecting the continuation of the entire operation especially in case of multi causalities incident resulting from an impact of infectious diseases.

Furthermore, for a successful emergency operation plan, I believe it is necessary for the management to be wide open for the creative solutions that might come from first responders who are responsible for stabilization of cases when they first encountered with them. The management should make monthly meetings to discuss what aspects can be developed to facilitate the process as well as to enhance the possibility of better outcomes. Finally, an effective emergency plan requires all hospital departments to work together and share resources and knowledge to properly mitigate, response, and recover from emergencies.
